What we collect

• Your name, email, practice name, and the practice-profile responses you submit.
• Your adopted policy records, including attestation name, title, IP address, and user-agent string at adoption.
• Operational logs (login times, page views, errors) and billing metadata.

What we don't collect

• We do not collect Protected Health Information (PHI). The Service is not designed for it and we do not offer a Business Associate Agreement.

How we use it

We use your information to operate the Service, generate your policies, send service-related notifications (e.g., template updates), and bill you. We do not sell your information.

Subprocessors

We rely on third-party infrastructure (hosting, database, email, payments). A current list of subprocessors is available on request.

Retention

We retain your policy records and audit logs for as long as you maintain an account, and for a reasonable period after cancellation to comply with our own legal obligations.
